Francis Marchal (born September 10, 1964) is a Swiss surgeon, a fellow of the American College of Surgeons, specializing in head and neck surgery and facial plastic surgery. He is a professor of Otorhinolaryngology Head and Neck Surgery (ORL-HNS) at the University of Geneva, Switzerland, and is responsible for the salivary gland unit at the Geneva University Hospitals (HUG). He is known for his contributions to the treatment of salivary gland diseases, particularly in the development of sialendoscopy.

Education

Marchal studied medicine at the University of Geneva, graduating in 1989. He worked in the emergency department under Kenneth Mattox at the Ben Taub General Hospital in Houston, Texas, where he completed his medical thesis on "Tracheo-esophageal penetrating wounds."

Initially specializing in cardiovascular surgery, he spent two years managing the **heart transplant program** at the University Hospitals of Geneva under Bernard Faidutti. He also trained with Michael DeBakey at the Texas Heart Institute.

Interested in facial plastic surgery and head and neck surgery, he trained in the **ORL-HNS Department** at the University Hospitals of Geneva under Pierre Montandon, in the **ORL-HNS Department** at the Inselspital Bern under Rudolph Häusler, and in the **Maxillofacial Department** with Joram Raveh.

In 1998, he was certified as a **Head and Neck Surgeon** by the **University of Geneva** and the **Swiss Federation of Physicians** (FMH). He also became a **Facial Plastic and Reconstructive Surgeon** certified by the **University of Lyon, France**, where he defended his thesis on "Nasal reconstruction with calvarial bone."

He obtained his Privat-docent title in 2003 at the **University of Geneva**, with a thesis on sialendoscopy. In 2011, he was appointed **Professor** at the **University of Geneva**, where he currently leads the **salivary gland unit** at the Geneva University Hospitals (HUG), focusing on clinical practice, research, and teaching.

Career and Accomplishments

In 1995, Marchal was sent by the **Swiss Society for ENT-HNS** to Germany to establish an **ultrasound training program** in Switzerland. In 1999, he organized the first Swiss ultrasound course, leading to the **official integration of head and neck ultrasound specialization** into Swiss ENT training.

During his time in Germany, he observed external stone fragmentation with a lithotripter and the use of urologic baskets for stone removal in the salivary ducts at the **University of Erlangen**, under **Heinrich Iro**. Upon returning to Geneva, he developed a minimally invasive instrument to explore the salivary ducts, leading to the advancement of **sialendoscopy**.

Marchal worked with the **Polytechnic School of Lausanne** to develop endoscopic prototypes, introducing specialized tools such as dedicated baskets for stone retrieval, dilation balloons, and reusable instruments. His early work, involving over 100 cases of parotid and submandibular sialendoscopy, gained international attention, including a feature in the **New England Journal of Medicine** in 1999.

In 1999, he organized the first **International Congress on Salivary Gland Diseases**, bringing together experts from multiple specialties. During this event, he also led the **first international course on sialendoscopy**, with contributions from pioneers such as **Oded Nahlieli** (Israel), **Dr. Katz** (France), and **Peter Gundlach** (Germany).

He later founded the **European Sialendoscopy Training Center of Geneva (ESTC)**, where he trained over 1,700 physicians from **58 countries** across **32 international courses over 24 years**.

Marchal has traveled extensively to conduct lectures and live surgical training, particularly in **India, Asia, and the Middle East**, supporting the growth of sialendoscopy worldwide.

In 2005, he co-founded the **Multidisciplinary Salivary Gland Society (MSGS)**, which became a founding organization of the **European Confederation of ORL-HNS**. He served as its **General Secretary for 12 years** and as **President for 8 years**, organizing international meetings.

In 2012, with increasing global interest in **sialendoscopy**, Marchal led the creation of the **International Sialendoscopy Society (ISIAL)**, which organizes global meetings on the topic. He currently serves as its **President**.

He was appointed by the **International Federation of ORL Societies (IFHNOS)**, the **American Academy of ORL-HNS (AAO-HNS)**, and the **American Head and Neck Society (AHNS)** to develop an **International Curriculum for Sialendoscopy**, recognized worldwide.

Contributions to Facial Plastic Surgery

In addition to salivary gland diseases, Marchal has remained active in **facial plastic surgery**. He became a member of the **American Academy of Facial Plastic Surgery** in 1997, the **French Society for Plastic Surgery** in 1998, and founded the **Facial Plastic Surgery Group of the Swiss Society of ORL-HNS** in 1999.

He has been involved in integrating **facial plastic surgery into cancer treatment** and advocating for its inclusion in **European medical training programs**. He contributed to the development of a **European training logbook and curriculum** for facial plastic surgery within the **European Union of Medical Specialists (UEMS)**.

Marchal also promoted the **facelift approach for parotid surgeries** and **muscular flap reconstruction** for salivary gland defects in both benign and malignant cases.

Awards and Recognitions

For his work in **salivary gland diseases**, Marchal received the prestigious **Eugene Nicholas Myers Lecture Award**, given to one international specialist annually for lifetime achievements in **Head and Neck Surgery**.

He also received the **Franz Escher Medal of Honor** from the **Swiss Society for ORL-HNS** and became an **honorary member** of several national ORL-HNS societies.

In 2025, Marchal is launching the **International Salivary Gland Diseases Foundation**, aimed at supporting research and training in minimally invasive salivary gland procedures worldwide.
