I'm afraid I'm going to have to fail this immediately per WP:GAFAIL item 3 ("It has ... cleanup banners that are unquestionably still valid") and WP:PAGENUM. This has been tagged for a full year for "cites its sources but does not provide page references". The vast majority of this article is sourced to the NTSB report which is 133 pages long. GA page numbers

Hello @RoySmith. Not that this is going to change anything but just to clarify, you stated that the article had been tagged for the inclusion of page numbers for a year.
